Long term prognosis of remaining dentition is not good. This patient will eventually need full-arch replacement. This eventuality may be delayed by removing the most problematic teeth and fabricating partial dentures. Remaining dentition can be maintained as best as possible (for now) with non-surgical periodontal therapy.
We discussed the importance of communication with both the patient and any involved specialists if proceeding with a "less-than-ideal" treatment plan. Shaun Whitney (periodontist) stated that, due to mobility and poor overall prognosis, he would be wary of surgical intervention if trying to maintain teeth. Systemic antibiotics may be beneficial.
- Main concern is missing front teeth and overall perio condition
- Patient smoked for 18 years before quitting
- Slight mobility- mostly on upper dentition
- Patient would like to avoid dentures
