This case was originally managed in 2018 using MTA. Recently, root resorption was noted near apex. Apicoectiomy was discussed as possible therapy. It is possible that this lesion is present due to MTA not reaching down to apex during original treatment.
There was also a discussion regarding dental materials, including bioceramic putty, which is the new standard of care for this procedure. A 4mm plug of putty is normally placed at apex, followed by complete obturation of canal. This is considered definitive treatment.
